Report an Injury

The First Report Of Injury (FROI) is the application used to initiate a workers' compensation claim and it is now available online for use by employers, medical providers and injured workers. You can submit the injury form via FAX or online.

If the injury involves a worker whose employer has University Hospitals CompCare as its workers' compensation managed care organization (MCO), you can print a blank FROI from this page, fill it out as completely as possible, and FAX it to University Hospitals CompCare at 1-800-654-3849. The free Adobe Acrobat Reader software is required to display and print the form.

Or, you can complete the FROI online and submit it electronically to the Ohio Bureau of Workers' Compensation (BWC), which will forward the appropriate information to the employer's MCO, including University Hospitals CompCare. When completing the online form, please use the Previous and Next buttons located at the bottom of the page to navigate through the form.
