Using Your University Hospitals CompCare ID Card

Q. Where do I find my University Hospitals CompCare ID card?

A. On the last panel of this guide. Please fill it out and always carry it with you. It verifies that you are covered for work-related injuries at your current place of employment by University Hospitals CompCare.

Q. How do I use the card?

A. You must present your ID card to your health care provider on every visit to your provider to ensure that University Hospitals CompCare is properly billed for your medical care.

Q. Can I use my ID card for non-work-related health care?

A. No. Your ID card is to be used ONLY when seeking treatment for work-related injuries. It may not be used by other members of your family or anyone else, and intentional misuse of the card could constitute fraud.

Q. Do I need my card when obtaining prescription medications?

A. Yes. You must present your ID card to your pharmacist for pre-scribed medications for your work-related injury. If the BWC has allowed your claim, you will not be charged for your prescription; there are no co-payments or deductibles. If you require prescription medication before your claim has been allowed, pay the pharmacist. When your claim is allowed, contact the pharmacist to initiate reimbursement. Pharmacies have a direct link to the agency, allowing them to find out if a claim has been approved as they are filling a prescription. Q. What if I lose my ID card?

A. Ask your supervisor or employer for a replacement ID card.

Q. Can I keep my ID card even if I leave my job?

A. No. If you leave your current place of employment, your coverage under University Hospitals CompCare ends. Please return the ID card to your supervisor.
